What is MyChart?

MyChart offers Meriter and Community Connect Partner patients personalized and secure online access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to:

MyChart is a free service offered to Meriter and Community Connect Partner patients and Physicians Plus Insurance members.

How do I sign up?

If you are 18 years of age or older and receive care from a Meriter Clinic or Community Connect Partner that offers MyChart, or have health insurance through Physicians Plus, you may activate an account.

  1. Sign up during your next appointment. Ask your nurse or provider for an activation code at your next visit.
  2. Request an account online – an activation code will be mailed to your home in 7 to 10 business days.

Who do I contact if I have further questions?

If you have questions regarding information in your medical record, please contact your clinic. They will assist you directly or connect you to someone who can help. Physicians Plus members may call Member Service at (608) 282-8900 with questions on their health insurance benefits.

Why is my e-mail address required?

Meriter, Community Connect Partners and Physicians Plus require you to provide a current e-mail address when you activate your account to ensure you are alerted when new information has been posted to your MyChart record or to any other record to which you have access. No medical information is sent as part of these e-mail alerts. You will receive an alert when a new test result, appointment reminder, message or other personal information has been posted to your MyChart account.

What is MyChartCentral and Lucy?

MyChartCentral makes it easy for you to securely access all of your MyChart accounts in one place using one username and password. Once you log into your MyChart account, you can connect to your other MyChart accounts that you setup with other health care organizations.

Lucy is your personal electronic health record that allows you to organize all your medical information. You can save your health record to a thumb drive and take it with you wherever you go. You can also add personal information about your health and choose to share it with your health care providers.

How do I sign up for MyChartCentral and Lucy?

  1. Log into your MyChart account.
  2. Under My Linked Records in the menu of the left, click Learn More.
  3. Follow the on-screen instructions.
  4. Check your e-mail for an activation message, and follow the activation link to get started!

What medical information is available in MyChart?

For patients of Meriter or Community Connect Partners, MyChart provides access to portions of your medical information stored in your medical record. This includes information such as your allergies, immunizations, prescriptions, laboratory results, diagnoses, appointments, demographics and billing information. You may also take advantage of online clinic services, such as scheduling appointments and sending secure messages to clinic staff.

MyChart does not display all information from your medical record, nor does it give access to every clinic service. Each web page will describe the information available to you. If you need information or services beyond what is available to you on-line, please call your clinic. If you need a complete copy of your medical record, please contact your clinic’s Health Information Management Department.

When can I see my test results in MyChart?

Your Meriter or Community Connect Partner test results are released to your MyChart record after your physician has reviewed them. This is generally within 1-2 weeks of your test, but will often be sooner.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor’s office. Please report missing or incorrect information to the clinical staff or your provider at your next office visit.

If I send a message to my doctor or nurse, when can I expect a reply?

MyChart allows you to send and receive secure electronic messages with clinical staff and providers who are active with MyChart. You will normally receive a response to clinic messages within 2 business days.

MyChart is not for urgent needs. If you experience a life or limb threatening emergency, call 911. If your need is urgent but not life threatening, call your clinic instead of sending a message.

Can I view a family member's MyChart account?

Yes; this is called Proxy Access. With the appropriate approvals, a parent (or legal guardian) can log into his or her personal MyChart account and then connect to information regarding his or her family member. If your child is age 0-11 you can be granted full access to your child’s MyChart record. If your child is age 12-17 you can be granted partial access to your child’s MyChart record. Privacy laws limit access to personal health data. Proxies of children age 12-17 are allowed to view allergies and immunizations, and can message their child’s providers and request appointments on their child’s behalf. Can I ask questions regarding a family member from my MyChart account?

No. Information you send is filed to your medical record. For user safety, it is important that you do not submit questions regarding other people from your account.

If you have proxy access on the behalf of another patient or member, please send messages regarding that patient or member from his/her account rather than your own account.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must have their own MyChart account.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into MyChart and from the left menu select “Preferences” to make the changes desired. You may also indicate if this change should apply to other members of your household or the billing account guarantor.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID’s, and passwords. Each person controls their password and the account cannot be assessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

I was logged out of MyChart, what happened?

We aim to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

My access code does not work, what should I do?

For your security, your access code expires after 60 days and is only valid for one-time use. Upon activation, this code expires and may be safely discarded. When you activate your account, you will create an ID and Password that only you will know. If you have trouble logging in or need a new access code generated, please call the MyChart helpdesk at (888) 256-3554.

Is my access code the same as my user ID?

No, your access code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 60 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.

Paragon Development Systems (PDS) provides 24/7 technical support for MyChart to ensure you are able to receive assistance at the time best for you. To reach a technical support representative, please dial (888) 256-3554.

PDS Representatives do not have access to your benefit and/or medical information.

How do I use MyChart on my iPhone or iPad?

You can use MyChart on-the-go via the iPhone, iPad or iPod touch! Once you activate your MyChart account, visit the iTunes app store to download the MyChart app, and then select Meriter as your provider. Please note, the MyChart app does not include health insurance information.
